To learn more, visit Role Scope:We are looking for an individual who is passionate about serving the underserved community. The Outreach Specialist will educate and build relationships with community partners, prospective participants, families, and/or caregivers to create awareness about Habitat Health's PACE (Program of All-Inclusive Care for the Elderly) program. The Outreach Specialists will also ensure a smooth enrollment process from outreach to enrollment for prospective participants.Core Responsibilities & Expectations for the RoleDevelop and maintain relationships with healthcare professionals, providers, Community-based organizations, senior housing facilities, post-acute facilities, and senior centers to drive referrals for the Habitat Health PACE programOrganize and conduct events in the community and at the Habitat Health Center to increase awareness of Habitat Health to the communityConduct presentations to promote the Habitat Health PACE programDevelop quarterly marketing plan to generate qualified enrollmentCollaborate with Center's management to develop retention strategiesIdentify new opportunities to enhance and improve the referral process and activitiesContribute to a program experience that Participants want to join, a team culture that cares and creates joy, and an environment where all participants and team members belongDrive the enrollment process as the case manager for prospective participants, including reviewing, processing, and updating applications and required documentation, distributing relevant enrollment materials, and managing communicationsConduct tele, home visits and in-center evaluations for the prospective participant for potential eligibility for care.Deliver high-quality customer service by addressing and resolving enrollment-related inquiries and concerns in a timely and professional manner.Maintain and record all productions/activities in CRM softwareStrictly adhere to DHCS and CMS regulations and policies regarding marketing practices for the PACE programFlexible to working events in the evening and weekends as neededOther duties as assignedRequired Qualifications: Experience in healthcare or enrollment sales, with a track record of generating business-business or business-to-customer referralsExperience working with State/County eligibilities workersProficiency in CRM software preferredExperiences working with Frail ElderlyHighly motivated individual with minimum supervisionS./B.A. Degree in healthcare, Business Administration, or Marketing preferredMust be able to lift and carry or otherwise move up to 25 poundsProof of current CPR/BLS certification required or requirement to obtain within 30 days of employmentProof of valid CA driver's license, personal transportation, good driving record and auto insurance as required by State lawPreferred Qualifications:Bilingual: Spanish/Mandarin/Cantonese preferredFamiliarity with the PACE program or other elderly care organizationsCompensation: We take into account an individual's qualifications, skillset, and experience in determining final salary. This role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected salary range for this position is $70,000 to $74,000 and is eligible for a monthly performance-based bonus.
