Our vision is revolutionizing how healthcare professionals produce, access, and utilize information and thus enabling them to focus on the core value of their work: patient outcomes.We are seeking a motivated and patient-focused Care Coordinator to support patients enrolled in Medicare's Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs.This is not a traditional back-office Medical Assistant role. Instead, you will serve as a key point of contact for high-risk patients with multiple chronic conditions - helping them stay engaged, compliant, and on track with their care plans through structured outreach and monitoring.Your Contribution:Make 50+ outbound patient calls per day using our phone systemUtilize multiple client EHR systems for documentationConduct monthly check-in calls for RPM and CCM patientsReview and monitor synced vital signs from RPM devicesOrder RPM devices and educate patients on proper usageConduct structured health assessmentsFollow personalized care plans tailored to each patientProvide education on nutrition, exercise, medication adherence, and lifestyle choicesTrack and document patient progress accurately and consistentlyYour Qualification:Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) requiredPreference given to National Certified Medical Assistants (NCMA)Minimum 2 years of healthcare-related experienceMinimum 1 year in related Chronic Care Mangement servicesStrong communication and interpersonal skillsComfortable with high-volume outbound callingStrong documentation and EHR proficiencyWhat you can expect from us:Purpose: Become a part of a significant initiative.
