Polaris Healthcare & Rehabilitation Center provides cutting-edge post-acute services, skilled nursing care and goal-oriented therapies in a homelike environment in charming and quaint Milford, Delaware.It is our mission to meet the physical, emotional and spiritual needs of our rehab clients and residents in an environment that is genuinely caring, while promoting dignity and respect.As a proud member of Nationwide Healthcare Services, Polaris is committed to delivering the highest standards of post-hospital care to expedite your recovery.Qualifications• High School diploma or equivalent• Strong oral and written communication skills• Ability to work effectively with small groups as leader or facilitatorResponsibilities• The Activities Assistant is responsible for planning and leading programs that bring purpose and meaning to the lives of the individuals who are residents or short term rehabilitation guests• Plan and run resident activities (arts, crafts, dance, music, exercise, brain games and sensory stimulation)• Create diverse programs to meet resident needs• Perform timely documentation• Ensure care plans are up-to-date and accurate• Keep clinical staff updated on changes to resident or guest condition• Safely transport residents and guests to programs• Encourage residents to participate in scheduled events• Arrange physical environment in preparation for activities to ensure resident and guest safety• Create a welcoming and festive environment for residents and guests• Promote and support culture of patient centered care and service
