Home Health RN Location: Hamden, CT Pay: $53.00/hour Position Type: Contract Work Setting: Field-Based / Home Health A-Line Staffing is seeking an experienced Home Health RN for an opportunity in Hamden, Connecticut . This role is ideal for a compassionate and organized Registered Nurse with strong home health experience, proficiency in Homecare Homebase and OASIS , and the ability to provide skilled nursing care in patients’ homes while coordinating closely with families and interdisciplinary care teams. Responsibilities Administer skilled nursing care to patients in their place of residence Assess patients in stable and emergent situations to identify physical, psychosocial, and environmental needs Develop, implement, document, and evaluate individualized plans of care Coordinate care with patients, families, physicians, interdisciplinary teams, and referring agencies Maintain accurate, timely, and thorough patient documentation Educate patients and families regarding medications, treatment plans, diet, home safety, and self-care practices Provide care using infection control measures and safe nursing practices Assess and manage patient pain while educating families on pain management techniques Supervise and provide clinical direction to home health aides and other support staff Arrange referrals to appropriate agencies and services when needed Monitor assigned cases for compliance with payer and documentation requirements Participate in team meetings and collaborate with healthcare professionals to ensure continuity of care Provide on-call support as assigned based on branch and staffing needs Qualifications Current Connecticut Registered Nurse license required Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program required BSN preferred Current CPR certification required Current driver’s license required Must have reliable transportation and insurance At least 1–2 years of RN experience in a clinical care setting required 2+ years of home health experience required Proficiency with Homecare Homebase required Proficiency with OASIS required CHPN certification preferred Skills Needed Strong patient assessment and care planning skills Knowledge of medication administration, venipuncture, and IV care Ability to manage wound care, catheters, drains, and durable medical equipment in the home setting Strong communication and patient education skills Ability to work independently and as part of a collaborative care team Strong organizational skills and ability to prioritize in a fast-paced environment Commitment to confidentiality, professionalism, and quality care Additional Information Local travel is required On-call responsibilities may be required based on staff and branch needs Expected productivity: 30 units weekly Benefits Benefits available to full-time employees after 90 days of employment 401(k) with company match available after 1 year of service Interested?
