HouseCalls Nurse Practitioner About the RoleHouseCalls Nurse Practitioners deliver in-home health assessments for Medicare Advantage members. In this role, you will visit members within an assigned geographic region to complete comprehensive evaluations. Visit types may include annual health assessments, post-discharge visits, or follow-ups with members who require more frequent support.Responsibilities Conduct in-home clinical assessments for health plan members, typically lasting 45-90 minutes. Complete all documentation electronically during and after the visit. Perform a comprehensive HouseCalls assessment, including: Past medical history review Review of systems Physical examination Medication reconciliation Depression screening Obtain and interpret vital signs; perform monofilament testing, urine dipstick testing, and foot exams as appropriate. Identify and document current and historical medical conditions using clinical judgment and assessment findings. Communicate relevant findings to the member's primary care provider to help close gaps in care. Identify opportunities for clinical referrals and escalate urgent or emergent concerns appropriately. Provide education on disease processes, medications, and treatment adherence. Maintain compliance with HIPAA and ensure the security of all protected health information (PHI). Required Qualifications Active, unrestricted Nurse Practitioner license with national certification (ANCC or AANP). No open or unresolved disciplinary actions, probation, or practice restrictions. Minimum 6 months of clinical NP experience at the highest level of education; clinical setting preferred. Certification in Family, Adult, or Gerontology. Experience in gerontology, cardiology, internal medicine, or endocrinology preferred. Home health or in-home visit experience is a plus. Skills and Competencies Reliable transportation to complete home visits. Strong administrative, organizational, and verbal communication skills. Professional presence and behavior. Ability to communicate effectively with geriatric and Medicare populations. Proficiency with electronic medical records and basic computer use. Ability to work independently with strong attention to detail. Dependable, reliable, and adaptable to varied home environments. Ability to lift a 30-pound medical bag and navigate stairs or public transportation as needed. Ability to sit, stand, and kneel to perform physical assessments. Additional Note No-Show Definition: A no-show occurs when the Nurse Practitioner is en route to the member's home or has arrived, and the member cancels the visit.
