Day to day as a Registered Nurse Case Manager As a Full-Time Registered Nurse Case Manager at Benevolent Cleveland Hospice Operations, LLC, your daily responsibilities will involve assessing patients' needs through comprehensive evaluations and developing personalized care plans that reflect their preferences and requirements. You will coordinate care with a multidisciplinary team, ensuring seamless communication among healthcare providers, patients, and families. A current RN license in the state of practice is essential, along with an Associate's or Bachelor's degree in Nursing, with a BSN preferred. Experience in hospice or palliative care nursing for a minimum of two years is critical, providing you with a solid understanding of end-of-life symptom management and family support.
