Day to day as a Registered Nurse Case Manager As a Full-Time Registered Nurse Case Manager at Benevolent Cleveland Hospice Operations, LLC, your daily responsibilities will involve assessing patients' needs through comprehensive evaluations and developing personalized care plans that reflect their preferences and requirements. You will coordinate care with a multidisciplinary team, ensuring seamless communication among healthcare providers, patients, and families. Daily tasks will include monitoring patient progress, adapting care plans as necessary, and providing education and support to patients and their families regarding treatment options and palliative care strategies. You will document all patient interactions and care plans meticulously to maintain compliance and uphold our commitment to excellence. What you need to be successful To thrive as a Full-Time Registered Nurse Case Manager at Benevolent Cleveland Hospice Operations, LLC, you will need a diverse set of skills and qualifications. A current RN license in the state of practice is essential, along with an Associate's or Bachelor's degree in Nursing, with a BSN preferred. Experience in hospice or palliative care nursing for a minimum of two years is critical, providing you with a solid understanding of end-of-life symptom management and family support. Proficiency in pain and symptom management, along with crisis intervention skills, is paramount to ensuring patient comfort and dignity. You should possess a deep understanding of Medicare, Medicaid, and hospice regulations to ensure compliance with state and federal guidelines.
