Responsibilities: • Coordinate and manage comprehensive hospice care plans tailored to each patient's unique needs. • Conduct regular home visits to assess patient conditions and adjust care plans as necessary. • Collaborate with interdisciplinary teams to ensure seamless communication and continuity of care. • Educate patients and families on hospice care processes, providing emotional support and guidance. • Document patient progress and care interventions accurately and promptly in electronic health records. • Advocate for patient needs, ensuring they receive the highest standard of compassionate care. • Participate in team meetings to discuss patient care strategies and share insights for improvement. Qualifications: • Experience in hospice or palliative care, demonstrating a deep understanding of end-of-life patient needs. • Ability to coordinate and manage comprehensive care plans, ensuring personalized and compassionate support for each patient. • Proven track record of conducting effective home visits, assessing patient needs, and providing timely interventions. • Strong collaboration skills to work seamlessly with interdisciplinary teams, fostering a supportive environment for patients and families. • Excellent communication skills to educate patients and families about hospice care processes, empowering them with knowledge and confidence. • Meticulous attention to detail in maintaining accurate and detailed patient records, ensuring seamless communication and continuity of care. • Advocacy skills to ensure patients' needs and preferences are respected in care decisions, amplifying their voices in the process.
