Job Title: Registered Nurse (RN) - Case Manager Location: Roseville, CA 95661 Contract Duration: 3+ Months (possible extension) Schedule: Monday–Friday | 08:00 AM – 04:30 PM (8-hour shifts) Pay Range: $65-$70 W2 Travel Pay: $2950/weekly Local and Travel applicant both are encouraged to apply! Required Qualifications: California RN license & BLS (AHA). 1 year of RN case management experience in Acute/hospital setting (Current). Care coordination, utilization review, and discharge planning. Key Responsibilities: Conduct comprehensive patient assessments and develop individualized care plans aligned with medical necessity. Collaborate with physicians, interdisciplinary teams, and external partners to ensure efficient care coordination and discharge planning. Monitor patient progress, evaluate outcomes, and adjust care strategies to enhance recovery trajectories. Facilitate utilization management while maintaining compliance with regulatory and organizational standards. Educate patients and families to empower informed healthcare decisions and promote long-term wellness.
