All candidates must cross credential at ProHealth Waukesha Memorial Hospital and ProHealth Oconomowoc Memorial Hospital. Days and Nights required Start in April/May for 7-10 shifts per month Waukesha 300 beds 24 ICU beds ADC 15 - 18 1 MD/1 APP during the day 0700 to 1900 1 APP at night 1900 to 0700 with MD on-call EMR EPIC CT Surgery IR Neurosurgery Neurology Nephrology 156 beds 11 ICU beds ADC 1 - 3 1 APP during the day 0700 to 1900 Waukesha night APP provides telemedicine support with MD on-call NP Requirements Summary Application Packet (appcentral), copy of government issued Photo ID (DL or Passport), past 2 years case logs in specialty requested, current CV, vaccination Records to include TB (2 skin tests within the past 12 months, one within 90 days of start or Quantiferon blood test done within 90 days of start), Hep B, Flu, Rubella, COVID (no longer required.
