Job Title: Home Health LPN-167985 Location: Lexington, KY 40509 · Service area includes Lexington-Fayette County & surrounding counties Pay: $37.30 per hour. Mileage reimbursement provided at $0.43 per mile . Schedule: Monday – Friday | 8:00 AM – 5:00 PM · On-call: 1–2 days per week + 1 weekend every 4 weeks Overview We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to provide skilled nursing care in a home health setting . In this role, the LPN will deliver individualized care to patients in their homes, ensuring adherence to care plans while promoting patient dignity, independence, and overall well-being. Key Responsibilities Provide skilled nursing care in accordance with individualized patient care plans. Monitor patient conditions, including vital signs, and report any changes or abnormalities. Administer medications, feedings, oxygen therapy, and perform treatments such as ostomy care as ordered. Support and follow care routines established by PT, OT, and Speech Therapy teams. Maintain strict infection control practices and report any potential infections or concerns. Accurately document all care provided, patient progress, and communication with care teams. Order and manage medical supplies, medications, and equipment as needed. Monitor and report incidents, ensuring proper documentation and follow-up care. Ensure medical equipment is functioning properly and coordinate repairs as needed.
