Home Health RN- PRN

Home Health Nurse Family Home Health Network, an affiliate of the Alden Network, is a provider of intermittent home health care, palliative care, hospice care, and skilled/custodial private care services throughout the Chicagoland area. We are seeking a dedicated home health nurse with a strong commitment to quality patient care. To succeed in this role, you must possess excellent clinical nursing and customer service skills. You will be responsible for completing a detailed initial assessment of the patient, the family, and the home environment to determine specific home health care needs, monitor and regularly re-evaluate the patients nursing needs, and initiate the appropriate plan of care as well as making necessary revisions as patient status and needs as they change. Minimum qualifications: Bachelor's degree in nursing (BSN) Must possess current Illinois RN licensure Home health experience preferred Current CPR Valid IL driver's license and auto insurance policy HCHB EMR experience a plus Must possess excellent clinical nursing and customer service skills If you enjoy being part of a dynamic team using your skills to have a positive impact on the lives of others, we would very much like to hear from you!
