Home Care RN Case Manager Now hiring an experienced RN Case Manager for 4 days per week of work in home care! (No OASIS) Ready to make a real impact in people's lives? Come grow with us and be a part of something special! This Case Manager should be available 4 weekdays each week. Our territory includes clients in the following communities: Maple Grove, Ramsey, Anoka, Blaine, Brooklyn Park, Big Lake, Osseo, Coon Rapids, Champlin, Andover, Elk River & Monticello. BrightStar RN Benefits: Weekly pay with direct deposit Benefits package including; health, dental and vision 401K match (up to 4% match) after 6-months of employment Paid time off Mileage reimbursement Free training and continued education Referral program Provide compassionate care to one patient at a time in home care Responsibilities of RN Case Manager: Provides care as defined within the scope of the state's nurse practice act. Makes the initial evaluation visit and regularly reevaluates the client's nursing needs. Prepares clinical and progress notes and summaries of care. Serve as backup for Critical Care Clients, In-School Staffing & PDN Assures proper maintenance of clinical records in compliance with local, state and federal laws. Helps educates and coach care-giving staff and assists with new hire clinical training, as needed. Participate in monthly clinical on-call rotation. No OASIS work. RN Case Manager Requirements: Current unrestricted RN license in Minnesota Active CPR Certification Negative TB skin test or QuantiFERON-TB Gold (QFT) blood test (completed within the last 90 days) Minimum two (2) years of clinical experience with one (1) year experience in a home health agency, primary care clinic or health facility, preferred. Critical Care Experience (1-2 years) Reliable car/auto insurance Valid Driver's License Clean background and criminal record. Understands regulatory compliance with state and federal regulations.
