Minimum Requirements Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active Oregon Physical Therapy license required to start the assignment. BLS certification from AHA or ARC may be required. Experience working in skilled nursing facility settings preferred. Experience with nethealth/PCC documentation system preferred. Client requires submission of resume to start credentialing process. Assignment Details Facility Type: Skilled Nursing Facility.
