Horizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, Horizon House has been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual/disabilities and homelessness to help stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout Southeastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice. We are seeking qualified Registered Nurse (RN) interested in joining our Intensive Case Management (ICM) team at our Claymont, Delaware location. Some of the responsibilities include the following. Assist in the development, implementation, evaluation and revision of recovery plans in collaboration with the participants and assigned staff. Maintains a case load of assigned participants and completes assigned paperwork in a timely manner. Act as a liaison consulting with community agencies and families to maintain coordination of care in the treatment process. Provides ongoing assessment of participant's mental health symptoms and responds to recovery plans. Educates and supervises participants in self administration of medication. Ensures annual review of the participants physical condition and assist access to and encourage compliance with treatment for identified physical problems. Monitors medication, documenting participant compliance, medication efficacy, and side effects as reported. Qualifications: Board Certified Licensed RN in the state of Delaware; or participating in the Nursing Licensure Compact to include the state of DE. MA/BA in nursing or a registered nurse with a Mental Health Certification from the American Nursing Association, preferred. Knowledge of mental health systems and supports. Knowledge of multiple support systems for substance abuse. Good communication skills both oral and written. Ability to work independently and as part of a focused team. Possess a valid driver's license with an acceptable driving record. Strong organizational and analytical skills. Scope: Frequent talking and listening. Requires the ability to lift, carry, hold, kneel, stack, stoop, reach and bend which may include direct intervention with participants. Local travel required, including transporting participants. May be required to report to work during emergencies, including inclement weather. May require crisis intervention in emergency situations. Scheduling flexibility and possible on-call work are necessary.
