Advanced Practice Provider - NP Heart Failure

We Are Hiring Ascension Saint Thomas Heart is seeking to add an Advanced Heart Failure APP to our team at our Heart - West Clinic in Nashville, TN. This is a great opportunity for the right candidate to partner with Ascension Saint Thomas to provide medical services to patients. Position Highlights: Minimum of two years hospital APP experience In patient service 5 heart failure specialists 6 APP's Manages care of LVAD, Heart Transplant and Advanced Heart Failure patients Rounds daily with MD's in ICU's and Specialty Step Down Unit Responsibilities: In collaboration with a physician, provide for the expansion of individualized primary healthcare services by diagnosing and treating certain acute or chronic health conditions. Obtain patient history and perform assessment via observation, interview and examination. Order, perform and interpret diagnostic studies. Perform preventative health assessments, screening, immunizations and patient care. Provide direct treatment and management of health conditions via referral to other healthcare providers or community resources. Counsel and educate patients and their families/caregivers concerning preventative health, treatment options and community resources. Document delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy. What You Will Need: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Nurse Practitioner credentialed from the Tennessee Board of Nursing obtained prior to hire date or job transfer date required. Licensed Registered Nurse credentialed from the Tennessee Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required.
