RN Hospice Case Manager Coverage Area: Sheboygan & Manitowoc Counties, WI Job Type: Crisis Rate | Contract Only Hours: 40 hours/week Shift: 8-Hour Day Shift | Mon-Fri (8:00 AM - 4:30 PM) Contract Dates: Feb 16 - May 16, 2026 Position Overview Facility is seeking an experienced RN Hospice Case Manager to support the Sheboygan County hospice team, with occasional coverage in Manitowoc County. This is a home hospice case management role managing a steady caseload of adult end-of-life patients across homes, ALFs, and SNFs. Caseload & Patient Population Caseload: 13-15 patients Setting: Home, Assisted Living Facilities, Skilled Nursing Facilities Patient Type: Adults only - End-of-Life / Hospice Ratio: 1:1 Required Experience 2.5+ years of hospice-based case management (REQUIRED) Prior home hospice experience Previous admission experience required EPIC Remote Client experience - REQUIRED Travel experience preferred First-time travelers accepted if hospice + EPIC Remote experience Required Licensure & Certifications Active RN License (Compact preferred) BLS Valid Driver's License & Auto Insurance Required Skills End-of-life care planning Pain & symptom management Wound assessment & care Care coordination Triage and clinical decision-making Strong hospice documentation skills Independent field-based practice Technology & Equipment EMR: EPIC Remote Client (Required) IV Pumps: Sapphire Schedule & Expectations Weekend Rotation: One weekend per month On-Call: One call shift per month Holidays: None Floating: Anywhere within Sheboygan & Manitowoc counties RTO: Manager approval required Orientation Up to 1 week, based on clinician comfort level Support Structure Hospice RNs Case Managers Manager on call for escalation/support Additional Notes Scrubs: Any color or business casual Parking: Community-based (field role)
