Job description: Oversee the daily operations of the Care Management Department, including care coordination, case management, discharge planning, utilization management, social work services, and pre-admission case management. Lead and support care management staff to ensure high-quality patient care and effective discharge planning. Establish and implement best practices, clinical standards, and departmental policies to improve care coordination and operational efficiency. Serve as a liaison between patients and families, physicians, healthcare teams, community agencies, and managed care organizations to ensure coordinated care delivery. Monitor department performance and ensure compliance with clinical, regulatory, and organizational standards. Collaborate with leadership to maintain financial integrity and effective resource utilization. Build and maintain relationships with skilled nursing facilities, community partners, and external healthcare providers to support smooth patient transitions. Qualifications: Masters Degree in Social Work (MSW) from an accredited program. Washington Clinical Independent Social Worker (LICSW) License required. 6+ years of experience in Case Management in an acute care setting. 2+ years of leadership experience at the Manager or Director level, preferably in case management within a large hospital. Strong leadership, communication, and team development skills. Experience collaborating with multidisciplinary healthcare teams and external care providers. Knowledge of care coordination, discharge planning, and utilization management practices.
