RN Case Manager - Home Health The Mary T Family of Companies has an immediate opening for a part-time Home Health RN Case Manager in North Branch, MN and surrounding area. The RN Case Manager is responsible for providing home health services and education to patients and their families along with the evaluation of patient needs through the development, monitoring, and updating of Plans of Care. The RN Case Manager trains, directs, and provides clinical supervision for Home Health Aides and other RNs and LPNs, and coordinates closely with related disciplines to ensure professional service delivery. Responsibilities include: Accepts home health referrals and completes initial assessment Regularly evaluates the care needs of the patient and make necessary revisions to the patient's plan of care Makes referrals to other disciplines as indicated by the patient's needs, or documents rationale for not doing so Teaches, supervises, and counsels the patient and patient's family regarding home health procedures, interventions and other care needs as appropriate to the patient's condition Supervises and teaches the home health aides about their responsibilities in meeting the patient's care plan needs Ensures that the documentation required for reimbursement of care is completed on a timely basis, and in compliance with State and Federal regulations Performs duties necessary to maintain quality standards Consistently facilitates positive interpersonal relations among staff Ensures service satisfaction through patient visits and communication with other patient nurses and caregivers On-call triage on a rotating basis for all home care patients Facilitates patient and caregiver independence to the extent possible Schedule: Daytime hours 8:30 AM - 4:30 PM, part-time hours available, 16 hours per week. All RNs participate in the On-call Rotation once every 4-5 weeks. Caseload: This position will have a maximum caseload of 25 patients, with LPN support. Requirements: Possess a valid driver's license with a good driving record and have reliable transportation The ability to pass a background check Strong verbal and written communication skills Experience with Oasis is preferred, but not required Basic computer skills RN License in Minnesota Required We can offer: $39 - $41/hr, depending on qualifications Paid Sick Time Off 401k with Company match Paid Training Annual Performance Evaluations with Opportunity for Merit Increases Educational scholarships Other discounts, perks and opportunities!
