Job Title: Occupational Therapist (OT) – Home Health Location: Portland, Oregon (West Portland & Surrounding Areas) Pay Rate: $2,520/week Contract Length: 13 Weeks Shift: Day Shift – 5x8s (08:00 AM – 4:30 PM) Guaranteed Hours: 40 hours/week Weekend/On-Call: No weekends, no call Job Summary We are seeking a Home Health Occupational Therapist (OT) for a travel assignment in Portland, OR . This role focuses on providing one-on-one patient care in a home health setting, helping patients regain independence and improve functional outcomes. Key Responsibilities Provide in-home occupational therapy services to adult and geriatric patients Conduct evaluations, reassessments, and develop individualized care plans Assist patients with ADLs, mobility, and adaptive equipment training Perform home safety assessments and discharge planning Deliver patient and family education for injury prevention and recovery Complete accurate documentation using EPIC and OASIS systems Collaborate with interdisciplinary teams to ensure continuity of care Requirements Active Oregon OT License (Required) BLS Certification (Required) Minimum 1 year of skilled Home Health experience OASIS documentation experience (Required) Must have reliable transportation (daily travel required) Preferred Qualifications Experience with EPIC charting system Strong background in rehabilitation, orthopedic, and neurological conditions Patient Population Adults and geriatrics Common diagnoses include: Stroke (CVA) Parkinson’s disease Dementia/Alzheimer’s ALS, MS Post-operative orthopedic cases Work Environment Home Health setting with 1:1 patient care Average up to 6 visits per day (30 productivity points) Daily travel radius up to ~60 miles (mileage reimbursed) Business casual attire (scrubs optional) Additional Details Facility provides efficient scheduling to minimize travel time Mileage reimbursed at IRS standard rate Facility may cancel 1 shift per pay period COVID vaccination + booster required This is a great opportunity for an OT looking to work independently while making a meaningful impact in a home health setting .
