RN CASE MANAGER

RN Case Manager Case Manager coordinates and procures services necessary to meet the post-acute care needs of patients identified as requiring such services. Ensures a timely and smooth transition to the most appropriate post-acute care setting. Works collaboratively with the interdisciplinary patient care staff both internal and external to the organization. Participates in quality improvement and evaluation processes related to the management of patient care. Ensures maximization of provider reimbursement of patient days through timely and accurate insurance reviews. Position requirements include alternating weekends. Education: BSN preferred License/Certification: Current New York State licensed Registered Nurse. Certification from the NYS Department of Health as a Patient Review Instrument (PRI) Assessor preferred. Experience: 3 years acute care clinical experience required; 5 years acute care clinical experience preferred. 3 years Case Management/Discharge Planning experience and/or 3 years utilization management experience required Skills and Certifications New York State RN License required PRI certification preferred 3 years of acute care clinical experience required 3 years of discharge planning experience and/or utilization management experience preferred Screening Questions Does candidate have a New York State RN License?
