Family Caregiver Support Coordinator Freedom Home Care - Milford, OH (In-Office Position) Full-Time About Us Freedom Home Care is a veteran-focused home care agency dedicated to providing high-quality, compassionate care to those who have served our country. We are growing quickly and looking for a motivated, organized, and people-focused professional to support and manage our family caregiver program. Position Overview The Family CaregiverManager plays a key role in supporting, managing, and guiding our family caregivers from onboarding through ongoing employment. This position ensures caregivers remain compliant, confident, and supported while maintaining high standards of care for our clients. This is a full-time, in-office position that requires strong communication, organization, and problem-solving skills. Key Responsibilities Onboarding & Hiring Support Assist with onboarding new family caregivers from start to finish Ensure all required paperwork and documentation is completed accurately Guide caregivers through systems (AxisCare, Viventium, etc.) Provide support and answer questions during the onboarding process Compliance & Documentation Monitor caregiver compliance with VA and company requirements Track required documents, renewals, and training completion Ensure all caregivers remain in good standing and audit-ready Follow up on missing or incomplete documentation Scheduling & Coordination Assist with caregiver scheduling and coverage needs as needed Help manage schedule changes, availability updates, and caregiver communication Support the scheduling team during high-demand situations Training & Support Provide ongoing support and guidance to family caregivers Assist with orientation and training sessions (weekly or as needed) Help caregivers understand care plans and expectations Reinforce company policies, professionalism, and best practices Caregiver Relations Serve as a primary point of contact for family caregivers Conduct routine check-ins to ensure satisfaction and performance Address concerns, questions, or challenges proactively Promote a positive, supportive caregiver culture Travel Requirements Travel may be required to client homes when additional hands-on support is needed for family caregivers Provide in-person assistance when issues cannot be resolved remotely Mileage reimbursement is provided for all work-related travel What We're Looking For Strong communication and interpersonal skills Highly organized with attention to detail Comfortable using computers, apps, and mobile technology Ability to multitask and adapt in a fast-paced environment Professional, dependable, and team-oriented Previous experience in healthcare, home care, HR, or caregiving (preferred, not required) Why Join Freedom Home Care?
