Job specification Candidate requirements Job specification Provide high-quality nursing care and education to Home Hemodialysis (HHD) patients, ensuring safe and effective treatment administration at home. Conduct thorough patient assessments, including physical, psychosocial, and lifestyle evaluations, to determine suitability for home dialysis. Develop individualized care plans and training programs to teach patients (and caregivers when applicable) how to operate dialysis equipment, follow safety protocols, and respond to complications. Monitor patient progress through home visits, telehealth check-ins, and collaboration with nephrologists and interdisciplinary renal care teams. Troubleshoot clinical or technical issues related to hemodialysis equipment and coordinate necessary maintenance or replacement. Candidate requirements Bachelor of Science in Nursing (BSN) required; advanced nursing education or certifications in nephrology preferred. Current Registered Nurse (RN) license in the state of Indiana. Minimum of 1-2 years of hemodialysis experience; previous home dialysis or peritoneal dialysis experience strongly preferred. Current Basic Life Support (BLS) certification required; ACLS preferred. Strong teaching, assessment, and communication skills to effectively guide and support patients in home-based self-care.
