Role Summary Our client, a healthcare provider specializing in dialysis and renal care services, is seeking a Home Peritoneal Dialysis Registered Nurse (RN) to provide clinical care and education to patients receiving peritoneal dialysis in a home setting. This role involves supporting patients and families through treatment management, monitoring patient outcomes, and collaborating with interdisciplinary healthcare teams to ensure high-quality care. The ideal candidate is a compassionate nurse with strong clinical expertise in dialysis care, excellent communication skills, and the ability to work independently while coordinating closely with care teams. Key Responsibilities Conduct patient assessments and develop individualized care plans for patients undergoing home peritoneal dialysis. Educate patients and caregivers on dialysis procedures, infection prevention, catheter care, and medication management. Monitor patient progress and adjust care plans in collaboration with the care team to optimize treatment outcomes. Coordinate with physicians, pharmacists, social workers, and other healthcare professionals to support comprehensive patient care. Maintain accurate and timely documentation of patient care and progress within electronic medical record (EMR) systems. Participate in quality improvement initiatives focused on improving patient outcomes and satisfaction. Maintain current knowledge of best practices, clinical guidelines, and standards related to peritoneal dialysis. Required Qualifications Active Registered Nurse (RN) license in the applicable state. Minimum of two (2) years of nursing experience. At least one (1) year of experience in peritoneal dialysis. Strong clinical knowledge related to dialysis care and patient management. Excellent communication and interpersonal skills. Ability to work independently in a patient home-care environment while collaborating with multidisciplinary teams. Proficiency with electronic medical records (EMR) and healthcare technology. Preferred Qualifications Bachelor of Science in Nursing (BSN) preferred.
