Case Manager - RN

The Case Manager-RN responsibilities include: The Case Manager Registered Nurse interacts with patients, physicians, and other members of the healthcare team to assure appropriateness of patient placement and education. They will provide outpatient and telephonic education, monitoring, and appropriate interventions to identify high risk inpatients. They will encourage appropriate utilization of services. They will promote timely discharge. They will provide education and in-services to members of the healthcare team when necessary. They will prepare and furnish reports to hospital committees and leadership. They will participate in problem solving activities They will perform other duties as assigned. Position details: Location: Morton Plant Rehabilitation Hospital - Clearwater, Fl Status: full time Monday - Friday 8:00 AM - 4:30 PM Occasional weekend requirement On Call: No Certifications and Licensures: Required RN (Registered Nurse) License Florida Required BLS (Basic Life Support) Preferred Case Management Certification Education: Required Associate Degree in Nursing Preferred Bachelors Degree in Nursing Experience: Required 1 year Utilization ~ OR~ Required 1 year Acute Care ~OR~ Required 1 year Home Care Preferred 3 years Nursing.
