Current Physical Therapy licensure in Illinois required to start the assignment. Current BLS/CPR certification required. Valid driver’s license, current vehicle insurance, and access to a dependable vehicle for patient visits. Experience providing home health physical therapy, including patient evaluation, development of plan of care, and home-based interventions. Ability to complete documentation per CMS and agency requirements, including timely submission of evaluation reports and progress notes. Schedule flexibility to participate in weekend rotation and potential availability on observed holidays as needed. Preferred: Experience supervising PTAs and home health aides where permitted by state regulation. Preferred: prior travel or agency-based home health experience.
