JOB SUMMARYThe Home Health Aide provides various personal care and related paraprofessional services in accordance with an established plan of care. Provides for the personal needs and comfort of patients in their homes.KEY ACCOUNTABILITIESCustomer Focus:• Regularly monitors customer satisfaction, striving to provide an extraordinary client experience• Performs personal care activities contained in a written assignment by the supervisor, which includes but is not limited to, assisting the patient with activities of daily living such as:o Personal hygiene (toileting, bathing, care of mouth, skin and hair)o Ambulationo Eatingo Dressingo Shavingo Meal Preparation• Performs other assigned activities which are taught by a nurse, for a specific patient, which includes but are not limited to:o Assisting with the change of ostomy applianceso Reinforcement of dressings (non-sterile)o Assisting with lifting and transferso Assisting with the use of devices for aid in daily living (cane, walker, wheelchair or lift device)o Assisting with prescribed range of motion exerciseso Assisting with self-administered simple urine tests for sugar acetone or albumin (may not interpret results)o Measuring and preparing special dietso Measuring intake and outputo Taking vital signs (temperature, pulse, respirations and blood pressure)o Application of an external (condom-type) cathetero Assisting with application of arm sling, arm splint or leg splintLikability/Passion:• Builds and maintains trusting relationships with clients• Warm, sensitive, compassionate, and puts clients at ease• The Home Health Aide performs light housekeeping tasks that will facilitate the patient’s self-careat home. These tasks include, but are not limited to:o Vacuuming or dustingo Sweeping or mopping floorso Doing disheso Laundryo Cleaning bathroomsResourcefulness/Initiative:• Innovates techniques to gain client interaction and improve satisfaction• Always looks for a way to improve how they perform their job and make the lives of their clients better• Passionately finds ways to overcome barriersJudgment/Decision-Making:• Demonstrates consistent logic, rationality and objectivity in decision-making• Achieves balance between quick decisiveness and slower, more thorough approaches• Always puts the client’s welfare firstListening:• “Tunes in” and listens to the opinions, feelings and needs of clients• Has the ability to be attuned to the client’s needs, even if they are unable to communicate• Understands the impact of one’s behavior on othersIntegrity:• Remains consistent in terms of what one says and does in terms of behavior toward others• Maintains confidentiality about the patient and patient’s family unless divulging information is essential for the safety, well-being, and protection of the client• Does not ethically cut cornersAccountability:• Fosters high levels of accountability by adhering to Right at Home policies and procedures• Performs work under the supervision of the Staffing Coordinator/Operations Manager• The Personal Care Assistant is responsible for written documentation of service provided, using the Client Care Record• Immediately reports any changes in the clientVerbal Communication:• Able to communicate crucial details regarding the client’s care, in clear and simple terms• Keeps Right at Home and all applicable parties informed• The Home Health Aide should not contact the physician directly, but rather refer all questions or reports to the nursing supervisor or appropriate partyAdaptability:• Adjusts quickly to changing priorities and client needs• Approaches stubborn or uncooperative clients with patience and flexibility• Not rigid in emotional or personal capacitiesFirst Impression:• Creates positive first impressions through appropriate body language, eye contact, posture, voice qualities, and attire• Does not let personal issues get in the way of professional responsibilities• Professional and kind in demeanor and approachThe Home Health Aide will NOT perform any of the following:• Perform heavy duty household cleaning such as washing walls, waxing floors, washing windows, etc.• Household repairs or maintenance• Yard care or maintenanceThe Home Health Aide will NOT perform any of the following medical cares:• Sterile dressing change• Irrigation of body cavities such as enemas, colostomy or wound• Gastric lavage or gavage• Catheterization• Administration of oral medications, eye drops or medicated skin ointments• Administration parenteral or IV medications• Application of heat by any method• Naso-gastric or gastrostomy tube feedings or care• Tracheostomy tube care• Suctioning of the nose, mouth or tracheostomy• Nail care on a patient with diabetes or compromised circulation• Any personal health service that has not been included on the paraprofessional task sheetNON-ESSENTIAL FUNCTIONS• Performs other duties as directed by the supervisor consistent with this job description and related toclient’s independent living where the client cannot perform such duties due to the client’s health or agelimitations.EDUCATION / SKILLS / ABILITIES / AVAILABILITY• High school graduate of G.E.D. certificate preferred• Six months experience as a Home Health Aide in health care (in homes or facilities) and successfulcompletion of a State approved Nurse Aide or Home Health Aide course• The Home Health Aide shall have successfully completed a Competency Evaluation that meets the Statelicensing law for home health agencies• Must have a sympathetic attitude toward the care of the sick, and ability to deal effectively with thedemands of the job• Current CPR Certificate is recommended• Meets the requirements for employees providing direct patient care• Read, write, speak and understand English as needed for the job• Has a valid driver’s license and use of an insured automobile for work or access to adequate transportation
