Exciting Opportunity for a Case Management MSW!Job Description:-Start Date:ASAP -Shift:5x8 Days (Monday - Friday, 0730 - 1600; hours may flex to 0800 - 1630) -Role:Clerical support for the Case Management Department - Manage authorizations and post-acute referrals - Deliver Medicaid notices to patients - Utilize Epic for charting and documentation -Patient Demographics:- Adults and Geriatrics - Average case load of 18-24 patients, with weekends up to 30 patients -Key Responsibilities:- Coordinate services for home care and equipment following discharge - Collaborate with internal and external team members - Document pertinent information to facilitate care coordination - Refer patients to rehab and long-term care facilities - Educate patients and caregivers to support discharge processes -Education and Experience:- 1 year of experience required - Clinical background in acute care is helpful but not required - Experience with Epic is mandatory -Supportive Environment:- Interdisciplinary collaboration with Physical Therapy, Social Services, Pharmacy, etc. - Orientation hours: 40 -Pre-Approved Time Off:1 request -Guaranteed Hours:72 hours per 2-week periodWhy Catalytic Solutions (CatSol):At CatSol, we connect dedicated therapists with rewarding assignments across the country.
