This is a rewarding opportunity to make a meaningful difference in the lives of our clients by providing personalized care and support.Responsibilities Include: Conduct skilled nursing visits in patients' homes (assessment, reassessment, medication administration, IV therapy, and treatments)Develop and implement individualized care plansEducate patients and caregivers on managing conditions and medicationsCollaborate with a multidisciplinary team to ensure continuity of careDocument visits and patient progress accurately and timely What do I need for this role with Community Care? Current Registered Nurse (RN) license in the state of Pennsylvania.Graduate of an accredited nursing program.Minimum 1 year of clinical nursing experience (Home Health experience preferred)CPR certification (required).Reliable transportation and valid driver's license.Compassionate, dependable, and able to work independently in a home setting.
