Direct Hire – RN Care Manager Schedule: Full-Time | Mon–Fri | 9:00 AM – 5:00 PM | Weekend rotation (1 Sat & Sun per month) Salary: $126,160 – $160,090 Benefits: Full benefits package + retirement plan Position Overview Seeking an experienced RN Care Manager to perform comprehensive patient assessments, coordinate multidisciplinary care, and support safe discharge planning. The role focuses on identifying high-risk patients, preventing readmissions, and ensuring appropriate utilization of healthcare services while collaborating with physicians and the care team. Key Responsibilities Conduct initial and ongoing patient assessments Identify high-risk patients and coordinate supportive services Manage care coordination and discharge planning Perform Utilization Review (UR) activities and payer reviews Arrange post-discharge care and patient education Collaborate with healthcare teams to ensure safe patient transitions Requirements BSN required Active NY RN License 3+ years clinical experience (ED, Critical Care, or Med/Surg preferred) PRI Certification (within 3 months of hire) Strong computer and documentation skills Preferred: Screen Certification, Case/Care Management certification (CCM/CMC), prior case management experience, and familiarity with Milliman/InterQual criteria .
