Social Worker (MSW) Location: New York, NY (Field-Based Role) Schedule: Monday–Friday, 9:00 AM – 5:00 PM Position Overview We are seeking a compassionate and detail-oriented Social Worker (MSW) to support a diverse client population through comprehensive care coordination and oversight. This role involves regular in-person visits in homes, facilities, and community settings to assess client well-being, ensure safety, and connect individuals with appropriate services. The ideal candidate is highly organized, adaptable, and committed to maintaining high standards of care and documentation. Key Responsibilities Manage an assigned caseload and ensure consistent follow-up and engagement. Conduct regular in-person visits with clients in their homes, residential facilities, or community settings. Assess client well-being and evaluate living environments to ensure safety and appropriateness. Identify client needs (e.g., home care, supportive services) and communicate findings for coordination with approved vendors. Monitor ongoing client situations and provide timely updates on status, risks, and outcomes. Collaborate with internal teams to ensure services are properly arranged and implemented. Refer clients to external medical, psychiatric, or specialized providers as needed (no direct treatment provided). Maintain accurate, detailed, and timely case documentation in compliance with agency and regulatory standards. Prepare reports and provide court testimony when required, including clear documentation of client conditions and actions taken. Ensure adherence to all agency policies, legal requirements, and ethical standards. Required Qualifications Master’s degree in Social Work ( MSW ). Prior experience in a hospital or nursing home setting. Ability and willingness to conduct in-person visits in client homes, facilities, and community settings. Preferred Qualifications Strong organizational skills with the ability to manage multiple cases and priorities. Excellent interpersonal and communication skills. Schedule & Flexibility Monday–Friday, 9:00 AM – 5:00 PM Flexibility required to respond to emergencies as needed Why Join Us?
