RN Field Case Manager Glasgow, KY 42141 (Field-Based / Home Health Setting) $56.14/hour Full-Time | Monday–Friday, 8:00 AM – 5:00 PM About the Role A-Line Staffing is seeking an experienced and compassionate RN Field Case Manager to provide comprehensive care coordination for high-risk patients. This role is ideal for a nurse who excels in clinical assessments, care planning, and collaboration with interdisciplinary teams to drive positive patient outcomes. Coverage Area Allen, Barren, and Hart Counties Key Responsibilities Conduct comprehensive assessments for high-risk members Develop and implement individualized care plans with primary care providers Continuously evaluate and adjust care plans based on patient needs Coordinate non-medical services such as transportation and housing Facilitate care transitions and collaborate with interdisciplinary teams Engage specialty resources to improve patient outcomes Maintain accurate documentation of clinical, functional, and financial outcomes Identify opportunities for health promotion and disease prevention Intervene promptly to prevent adverse patient events Advocate for patients while maintaining privacy and compliance standards Schedule & Workload Full-Time (40+ hours/week) Monday–Friday, 8:00 AM – 5:00 PM On-call: Every 3rd weekend and 1–2 weeknights Productivity expectation: 30 points per week Qualifications Active, unrestricted Registered Nurse (RN) license in Kentucky Active RN Compact License required Associate’s or Bachelor’s Degree in Nursing (or related field) Strong analytical and clinical decision-making skills Case Management certification preferred Preferred experience: Homecare Homebase (HCHB) Wound care IV therapy Why Apply?
