Certified Home Health Aide (CHHA) Full-Time | Field-Based Vitality Hospice & Palliative Care is seeking a compassionate and dedicated Certified Home Health Aide (CHHA) to join our growing team. Position Details Schedule: Full-Time, MondayFriday, typically 7:00 AM3:00 PM Occasional evening/weekend overtime available Field-based position with travel required (mileage reimbursed) Active CHHA certification required What You'll Do Working under the supervision of a Registered Nurse Case Manager, you will provide essential hands-on care and support, including: Assisting with mobility, transfers, and ambulation Providing personal care (bathing, grooming, dressing, feeding) Offering companionship and emotional support Performing light housekeeping tasks Documenting care accurately and in a timely manner Supporting a culture of compassion, quality, and respect You will be an integral member of our interdisciplinary team focused on clinical excellence and patient-centered care.
