Job Description Job Description POSITION TITLE: Licensed Practical Nurse (LPN) - PRN REPORTS TO: RN / Administrator POSITION SUMMARY: Woodland Hills Assisted Living is seeking a dependable and clinically strong PRN LPN to support our nursing team. This position is responsible for providing high-quality, resident-centered care including medication administration, monitoring resident conditions, documentation, and supporting caregivers during assigned shifts. This role is essential in maintaining compliance with West Virginia assisted living regulations while promoting dignity, safety, and aging in place for all residents. COMPENSATION AND SCHEDULE:Competitive hourly pay based on experiencePerformance-based bonus opportunities (attendance and facility census incentives)Flexible PRN scheduling - pick up shifts based on your availabilityPreferred availability: minimum of 2-4 shifts per monthConsistent attendance required to remain active in PRN pool RESPONSIBILITIES AND ACCOUNTABILITIES:Administer medications and treatments in accordance with physician orders and facility policiesMonitor residents for changes in condition and report to RN, provider, and family as appropriateComplete accurate and timely documentation including progress notes and incident reportsFollow up on new physician orders, labs, and appointmentsAssist with admissions, move-ins, and resident orientationSupport and implement resident service plansSupervise and support caregiver staff to ensure quality care deliveryMaintain compliance with all WV assisted living (OHFLAC) regulationsFollow infection control, safety, and emergency proceduresCommunicate effectively with residents, families, and interdisciplinary team membersPromote a respectful, compassionate, and resident-centered environmentPerform additional duties as assigned by RN or Administrator QUALIFICATIONS:Active LPN license in the State of West Virginia (required)Graduate of an accredited school of nursingExperience in assisted living, long-term care, or geriatrics preferredAbility to work independently and make sound clinical decisions JOB SKILLS:Strong knowledge of medications, treatments, and nursing care practicesAbility to assess and respond to changes in resident conditionStrong documentation and organizational skillsEffective communication and teamworkAbility to supervise and support caregiversKnowledge of assisted living regulations preferredAbility to remain calm and effective in urgent situations WHY WOODLAND HILLS:Family-owned and operated environmentSmall, close-knit teamFocus on quality care and meaningful resident relationshipsSupportive leadership and positive work cultureOpportunity for consistent PRN hours and potential growth
