Travel RN Case Manager Heartline Staffing, in partnership with Purple Cow Recruiting, is seeking an experienced Travel Registered Nurse Case Manager for an acute care hospital assignment in Mission Viejo, California. This role supports inpatient care coordination and discharge planning for medically complex adult and geriatric patients in a structured hospital environment. POSITION OVERVIEW The RN Case Manager is responsible for coordinating patient care across the continuum, facilitating safe and timely discharges, and collaborating with interdisciplinary teams including physicians, social services, and external providers. ASSIGNMENT DETAILS 13-week contract Day shift | 5x8s (08:0016:30) Weekend rotation required Patient ratio: approximately 1:25 Acute care hospital setting Epic EMR Floating required to Laguna facility COMPENSATION Gross: $3,059 per week W2: $28.00 per hour Tax-Free Stipend: $1,939 per week CORE RESPONSIBILITIES Coordinate inpatient care and discharge planning Collaborate with physicians, social services, and payors Arrange DME, SNF placement, and transportation services Ensure compliance with payer and regulatory guidelines Manage patient throughput and length of stay Document accurately in Epic EMR WORK ENVIRONMENT Acute care hospital setting Adult and geriatric patient population High-volume case management workflow Independent clinical decision-making required REQUIREMENTS Minimum 2 years acute hospital Case Manager experience REQUIRED (HARD STOP) MUST have inpatient care coordination and discharge planning experience Travel experience REQUIRED (HARD STOP) Epic EMR experience REQUIRED Active California RN license REQUIRED (pending accepted, must be active prior to start) Ability to arrange DME, SNF placement, and transport REQUIRED Ability to manage ~1:25 caseload REQUIRED Weekend rotation required Important Notes: Utilization review-only experience NOT accepted RTO limited to 7 days Local candidates accepted
