Treasure Coast Center for Surgery (TCCS) Multi-specialty surgery center SCHEDULE / SHIFTS - 5 days per week 40 hours per week - MondayFriday, 7:00a3:00p (plus lunch break) - No weekends - Minimum shifts per month: 20 FACILITY / COVERAGE - 7 ORs, 2 procedure rooms - MDs on-site supervise CRNAs - Paper charting (no EMR) - Daily case volume: 4580 CASE MIX - General, Ortho, Plastics, ENT, GI, GYN, Eyes, Peds - 23 MDs on site - 810 CRNAs daily ASSIGNMENT DETAILS - Duration: 3 months - Setting: Outpatient IMPORTANT REQUIREMENT - MUST be willing to cross-credential and work at the surgery center in Jensen Beach in addition to TCCS - Jensen Beach will require some pediatrics PRIVILEGES - 60 days (clean files only) SUBMISSION REQUIREMENTS - Active FL State Medical License - Anesthesia Modality Checklist completed by the provider within the last 6 months - CV in complete month/year format covering all gaps of 30 days or more - NPDB Self-Query required for providers who have malpractice claims or background issues - Completed EVPS disclosure form - Clinical interview may be required RESTRICTIONS - No providers who worked for Envision within the past 12 months AND live within 75 miles of the site - No providers who worked for this site or an affiliated site in the past 24 months - No providers that worked at this site or an affiliated site within 75 miles of this site - New candidates who have never worked for Envision must live outside the 75-mile radius - Pay rates cannot be advertised in any format AVAILABILITY QUESTIONS (COLLECT FROM PROVIDER) - 1st possible start date? - How many shifts per month can they cover? - What month is their schedule completely open? - Any shift preferences (day, night, blocks, etc.)? - Any dates in the next 36 months they cannot work? - Any holidays in the next 36 months they would be open to work?
