Job Description

Clinical Care: Complete admission, routine, PRN, and death visits for patients in a community or small facility setting. Case Management: Coordinate communications and actions across various disciplines to ensure the health and safety of persons served. Documentation: Maintain accurate daily, monthly, and annual nursing documents, including progress reports to physicians and the transcription of medical orders. Medication Management: Oversee the ordering, monitoring, and disposal of medications as prescribed, ensuring strict adherence to safety policies. Supervision & Training: Direct and supervise LPN/LVN activities and provide health-related training to unlicensed staff and new employees. Compliance: Ensure medical equipment and supplies are maintained within infection control guidelines. Providing nursing care to hospice patients Admit patients eligible for hospice services Delivering high quality, compassionate hospice, oncology and palliative care for home hospice patients Provide hospice and palliative care at home Delivering high quality, compassionate hospice, oncology and palliative care for home hospice patients Paid for 40 hours to provide hospice and palliative care at home Giving or assisting with personal care of hospice patients Processing initial referrals into hospice Provide direct patient care services to hospice patients and supportive care to the patients and family per the hospice plan of care Support hospice and level of care appropriateness Develop and implement home health care and hospice budgets Provide nursing services to patient/family utilizing the hospice concept Represented in the members of the hospice care team Provide nursing care to patients Perform supervisory visits with the hospice team Coordinate the implementation of each hospice plan of care Complete documentation on hospice aide supervisory notes Provide home nursing care instructions to patients Evaluate patients for admission to hospice house and perform admissions Maintain and develop knowledge of evolving hospice and palliative care philosophy and practices Education: High School Diploma or GED (verifiable) Attendance: Commitment to mandatory attendance during the first 90 days. 1+ year of Hospice RN experience is required. Software Proficiency: Prior experience with Homecare Homebase (HCHB) is required. Prior experience with Oasis is required. Clinical Skills: Demonstrated experience completing admission, routine, and death visits. Licensure: Active RN License. Certifications: Valid BLS CPR Certification. Transportation: Valid Driver’s License and active car insurance (required for commuting across York and Lancaster counties).
