Active Oregon Physical Therapy license required to start the assignment. BLS certification from AHA or ARC may be required. Home Health experience preferred. Experience with HomeCare Homebase EMR preferred. Must be able to perform PT visits including SOC, PT evaluations, recertifications, ROC, and supervisory visits. Ability to cover Jackson County service area and travel between patient homes. Mileage reimbursed between visits from first patient to last patient. Assignment Details Facility Type: Home Health. Shift Name: Days. Shift Details: Mon-Fri, 0800-1630. Weekly Hours: 40 hours per week guarantee. Productivity expectation: 30 points. Coverage Area: Jackson County.
