Schedule + Contract Details:Start: ASAP or within 2 weeks of OfferLocation: Mission Viejo (Orange County)Shift: Monday - Friday Thursday- TuesdayNote: There are several openings with varying schedules & territory options Contract: 13 Weeks to Start High Chance of ExtensionHours: with productivity model 27.5 - 30 units per week!On Call Requirements: Requested if needed, as neededCompensation: Up to $131,020 per year PLUS mileage reimbursement & full benefitsResponsibilities:Complete initial & comprehensive assessments to determine patient eligibilityCreate and implement individualized care plans with the patient s providerUse sound clinical judgment and document accurately via HCHB & OASISCommunicate regularly with the care team, scheduling, and leadershipBe the calm in the chaos delivering compassionate, professional care while keeping the workflow moving efficientlyRequirements Qualifications:2+ years working in Home Health setting visiting multiple patients per day!
