Job Description

ORGANIZATION The mission of Ravenswood Family Health Network (RFHN) is to improve the health of the community by providing culturally sensitive, integrated primary and preventative health care to all, regardless of ability to pay or immigration status, and collaborating with community partners to address the social determinants of health. POSITION SUMMARY Under the general direction of the Integrated Behavioral Health Services (IBHS) and Social Services Director and the Senior Operations Manager for Patient Advocacy, the Licensed Marriage and Family Therapist (LMFT) is an essential team member in the IBHS program providing consultations, assessments, and time-limited family-centered services. Additionally, the LMFT helps develop, nurture, and sustain community resources for patients and families, and works together with IBHS team members in planning and implementing strategies to improve the overall health and well-being of our patients and families. HOURS Part-time, 16 to 32 hours per week DUTIES AND RESPONSIBILITIES To be performed in accordance with RFHN Policies and Procedures * Completes brief psychosocial assessments and provides treatment to adults and/or children in English and/or Spanish. * Provides a range of therapeutic interventions, including but not limited to, crisis intervention and supportive counseling, bereavement support, behavioral health screens, and follow-up sessions for patients. * Accepts clinical warm hand-offs as requested by medical providers and support staff. * Consults, develops, and implements a psychosocial treatment plan in collaboration with interdisciplinary team members as appropriate. * Provides brief psychological interventions focused on stabilization and harm reduction, using evidence-based, short-term practices to address a range of psychological symptom presentations. * Assesses, reports, and provides interventions in cases of domestic violence, child, dependent, adult, or elder abuse, and other reportable situations in accordance with ethical practice, legal mandates, and RFHN's policies. * Tracks patients' progress and communicates with them as necessary to support their social and emotional well-being. * Documents all patient interactions into the Electronic Health Record (EHR), and completes and submits all notes according to organizational policies. * Attends department, health center, and community in-services related to field of practice and organizational requirements. General Agency Duties * Fosters an environment that promotes trust and cooperation among patients and staff; maintains strictest confidentiality concerning all patient and employee information. * Promotes and exemplifies in action the mission, goals, policies, procedures, and principles of RFHN to patients and other clinic staff. * Attends RFHN staff retreats and meetings, as supervisors requested. * Performs other duties as assigned and requested.
