This role is responsible for conducting comprehensive diagnostic assessments, determining appropriate levels of care, and ensuring a smooth transition into services.This position is vital to ensuring clients feel heard, supported, and connected to the right resources from day one.Key ResponsibilitiesClinical Assessment & IntakeConduct initial mental health and substance use diagnostic assessments using evidence-based tools.Evaluate client history, presenting concerns, risk level, medical needs, and social determinants of health.Determine level-of-care recommendations and collaboratively develop initial treatment plans.Facilitate crisis assessments as needed, including safety planning and referrals for higher-level care.Care Coordination & CommunicationCommunicate assessment results and clinical recommendations with clients, families, and treatment teams.Complete referral coordination for internal services (therapy, groups, psychiatry) and external supports when indicated.Ensure accurate, timely documentation in accordance with Medicaid, HIPAA, state, and federal guidelines.Participate in interdisciplinary staffing and treatment meetings to support client engagement and retention.Professional StandardsMaintain current knowledge of outpatient behavioral health regulations, TJC standards, and ethical guidelines.Uphold confidentiality, trauma-informed care principles, and culturally responsive engagement.Foster a welcoming, compassionate environment for individuals entering treatment.Education RequirementsBachelor's degree in Social Work with LSW and relevant clinical or assessment experience ORMaster's degree in Social Work, Counseling, or a related behavioral health fieldCandidate must be 21 years or older.Required LicensureValid, unencumbered state licensure:LSW, LISWLPC, LPCCMFT, LMFTService AreasOutpatient Mental Health & Substance Use ServicesProviding structured therapeutic interventions, case management support, and ongoing symptom monitoring for individuals needing routine or intensive outpatient services.Intensive Outpatient & Partial Hospitalization CoordinationCollaborating with higher levels of care to ensure seamless admissions, transitions, and follow-up support.Our Program ValuesSolero Behavioral Transitions is dedicated to helping individuals build:Resilience SkillsEmotional strengthSense of selfOptimistic outlookHealthy coping strategiesPractical Life SkillsTime and money managementProblem-solvingOrganization and follow-throughPersonal accountabilityCommunication SkillsActive listeningInterpersonal awarenessBoundaries & expressionSocial and emotional intelligenceThese foundations support long-term recovery and whole-person wellness.Solero Behavioral Transitions Where healing begins with understanding.
