Star of Care (SOC) RN, Home Health

Position Summary:Plan, organize and direct home health care services, from initial evaluation and assessment, pain and medication management, and care plan development.Ensure high quality patient experience and positive health outcomes through a comprehensive and personalized plan of care, proactively making revisions based on patient needs.
