PHMC is proud to be a leader in public health. PROGRAM OVERVIEW: Pathways to Recovery (PTR) is Philadelphia's short-term co-occurring disorder treatment partial hospitalization program. The program is funded by Community Behavioral Health (CBH) and Behavioral Health Special Initiative (BHSI) for Philadelphia residents with co-occurring substance use and mental health challenges. Our program works with participants 20 hours per week, for up to nine weeks. We provide a menu of treatment services available each day including individual, group, and family therapy; medication-assisted treatment and medication management with our psychiatrist; health care coordination and full-time nursing support; full-time case management; and full-time peer recovery specialist support. JOB OVERVIEW: We are seeking a Certified Peer Specialist (CPS) to join the Pathways to Recovery (PTR) team as a Peer Recovery Specialist. This individual will use their own lived experience to engage and support PTR participants. Schedule Requirement: Must be available to work evenings and Saturdays, rotating coverage with other Peer Recovery Specialists RESPONSIBILITIES: * Attend daily staff meetings and inform staff of any important participant information. * Implement the recovery process for participants as defined in Pathway to Recovery's program goals and philosophy. * Provide continuity of care as defined by the treatment plan for each participant. * Lead psychoeducational and self-help groups. * Engage participants by sharing personal story of recovery. * Provide outreach at treatment facilities, on the streets, and in participants' homes to ensure engagement during and prior to entering treatment. * Transport participants to initial intake, to and from the program, and for all necessary appointments. * Take participants to community-based self-help meetings such as AA, NA, etc. * Sit in with participants during recovery planning meetings as their advocate and resource. * Assist with discharge planning and maintain contact with participants for at least two weeks post-discharge. * Reengage participants on the streets and bring them back into treatment. * Provide input to the clinical team. Alert the team of any current or potential behavioral or health-related problems of participants. * Meet or exceed program productivity requirements, including ensuring daily census requirements and new participant enrollment. * Maintain a continuous record of all contacts, events, and progress toward the accomplishment of goals as stated in the treatment plan. Ensure that charts meet all standards. * Record individual and group progress notes in the DAP format after each therapeutic activity. * Complete reviews in compliance with state licensing and other program guidelines. * Assist the treatment team and participants with continuing care plans. * Perform other duties as assigned. QUALIFICATIONS: * Certified Peer Specialist (CPS) certification * High school diploma or GED * 1+ years of experience working or volunteering with individuals struggling with addiction and mental illness * Valid driver's license in good standing * Ability to drive a large van * Ability to establish trusting and therapeutic relationships * Commitment to recovery, choice, empowerment, and the ability of people with co-occurring disorders to find meaningful lives in the community SALARY GRADE: 13 PHMC is an Equal Opportunity and E-Verify Employer.
