The MD will provide patient care with team-based support at Cascadia's outpatient clinics, and Cascadia's residential or community sites as directed by the Medical Director. The MD will be principally responsible for oversite of team-based primary care delivery at a Cascadia Health Center, but will also provide patient care with team-based support at Cascadia's other clinics, residential or community sites as directed by the Medical Director. This position will meet regularly with the team of peer primary care providers, psychiatric providers and the Primary Care Medical Director to help design, implement and continuously improve our pioneering integrated primary care services. Part of the care provided by the MD will include Medication Supported Recovery (Suboxone) and psychiatric medications management for low acuity or stabilized clients. This position may share low volume after hours call rotation (triaged through a nurse call line) with the other primary care providers. Essential Responsibilities This position description is not intended to be an all-inclusive list of responsibilities, skills, or working conditions associated with the position. Provide primary care diagnosis and treatment integrated within Cascadia's behavioral healthcare services to clients with mental health or substance use disorders and patients assigned by our Coordinated Care Organizations.Lead the primary care staff in team based care integrated with behavioral health staff at the assigned clinic, working as a peer with psychiatric doctors and nurse practitioners at that clinic.Provide primary care consultation to Cascadia's psychiatric providers and behavioral health clinicians.Provide medications management and Medication Supported Recovery for low acuity or stabilized individuals in coordination with their psychiatric provider.Work with the integrated staff at a health center and other assigned sites in the development of wellness programs.Share after hours call rotation (triaged through a nurse call line) with the other primary care providers.Work with the Primary Care Medical Director and provider team to define goals and review the progress of integrated primary care operations.Regularly review and continuously improve quality of care provided by your clinic or care team through periodic chart review, quality dashboards, daily huddles, and rapid improvement cycles.Work with the agency Clinic Directors and facility support staff, in the spirit of stewardship, to help assure the agency systems and facilities continuously operate at high levels of efficacy and harmony.Demonstrate responsibility for safety of consumers and/or residents, staff and property; demonstrate familiarity with fire regulations and evacuation procedures.Maintain a high level of fluency in the use of the electronic health record.Deliver timely, accurate, and professional documentation.Adhere to mandatory abuse reporting laws and HIPAA requirements.Provide input on all relevant documentation and procedural requirements.Complete annual employee training requirements on a timely basis per agency requirements. Attend seminars, training and other educational opportunities in order to develop professional skills and abilities.Participate in all scheduled staff meetings, supervision sessions, and other departmental and agency meetings.Perform other duties as assigned.Qualifications Education: Requires an active state medical license in Adult Medicine or Internal Medicine MD or DO including maintenance requirements and DEA certification.Experience: Three years of clinical experience with primary care.Experience using an electronic health record (EPIC preferred).Experience serving communities of color and immigrant populations preferred.Experience with community health clinic, community mental health or homeless and special needs populations preferred.Leadership experience preferred.Specialized Knowledge, Skills and Abilities: Sufficient computer skills to complete documentation in an electronic medical records system in a timely mannerProvide care in a setting consistent with the principles of collaborative care informed by stages of change modelsOther: Ability to perform CPR, First Aid, and properly use AED equipment. CPR/ First Aid certifications are required and must remain current.Working Conditions Work is performed in a typical clinical setting. As such, the setting is not secure, and the LMP will be responsible for creating a safe environment within the area. In some cases, safety personnel may be available to assist with this goal. Primary care providers will be provided the appropriate tools with which to meet the requirements of their work (e.g.- computer, printer, scale, medical equipment and supplies, etc.). Meal periods and breaks are subject to interruption. Universal Precautions and remaining alert to the environment are critical to address the potential for violent behavior, exposure to disease, biohazards, noise and contaminants. Mental Demands: The MD must be able to handle a variable workflow, manage time effectively, prioritize activities, and help primary care staff with whatever tasks are necessary to establish this new primary care capacity. The work assigned is diverse and involves addressing new and unusual circumstances in which outcomes may negatively affect costs, employee morale or clients. The work regularly involves a degree of unpredictability and disruption of planned tasks requiring a flexible time management approach. Physical Demands: The MD must have the ability to assist the team in providing safety to all clients and staff in potentially volatile situations.
