Job Summary: The Clinical Pharmacy Specialist serves as a technical expert who assists in the development, coordination and implementation of clinical pharmacy services at Westchester Medical Center. Responsibilities: * Provides direct patient care through participation in multidisciplinary team rounds and collaborates with physicians in establishing drug criteria for the safe, economical and appropriate use of medications. · Disseminates pharmacotherapy knowledge through evaluation of literature, evidence-based practice recommendations and response to drug information questions. · Implements policies and procedures to ensure compliance with clinical practice and all applicable federal, state and local laws and regulations. · Supports systems-based care and population health to ensure proper safe dosing, storage, handling, dispensing and administration of medications. · Participates in institutional process improvements to ensure medication and patient safety · Manages non-formulary requests for drugs, and participates in Pharmacy and Therapeutics Committee activities. · Participates in other hospital Committees based on practice area. · Uses effective communication skills and builds effective working relationships with other healthcare providers. · Maintains professionalism and excellent conflict resolution skills. · Participates in continuing professional development. · Educates health care professionals, students, patients, and the public. · Participates in clinical development and educational initiatives within the Pharmacy Department. · Performs additional duties as required by the department. Qualifications/Requirements: Experience: PGY1 pharmacy residency and PGY2 specialty residency or three years of experience as a Clinical Pharmacy Specialist Education: PharmD Degree Licenses / Certifications: NY State Pharmacist License, required Board of Pharmacy Specialty Certification, preferred
