JOIN OUR GROWING TEAM Reports To: Team LeadPurpose: Under the direction of a Team Lead, the QMHP provides training to the client and natural supports that addresses the serious emotional disturbance and symptom-related problems that interfere with the client's functioning, provides opportunities to acquire and improve skills needed to function as appropriately and independently as possible in the community, and facilitates the client's community integration and increases his or her community tenure. This service includes treatment planning to facilitate resiliency. Qualifications: QMHP or qualified mental health professional ~ A staff member who is credentialed as a QMHP who has at least a bachelors degree demonstrated and documented competency in the work to be performed and has: A minimum of a bachelor's degree from an accredited college or university with a minimum number of hours that is equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention. A Registered nurse (RN) ~ licensed as a registered nurse by the Texas Board of Nursing in accordance with Texas Occupations Code, Chapter 301 General Responsibilities: The workers is responsible for providing medical necessary services to the client in accordance with the treatment plan that is designed to meet the mental health, educational, vocational, residential, financial, social and other needs of the client.Follow Texas Department of State Health Services, Resilience and Recovery Guidelines by completing reassessments: treatment plans, CANS/ANSA and ensure that annual assessments are completed on caseload Serve as an intricate member of the LES team to ensure continuity of care and supports the services 24 hours a day, seven days per week as necessary Provides structured face-to-face scheduled sessions to client based on the treatment plans Provide services in the home, school, office and community settingEnsure that the majority of mental health services are primarily rendered in the community and ensure that limited services remain clinic based. Documentation completed within 72 hours of serviceReport to supervision and in-house trainings as scheduledWork collaboratively with and provide ongoing updates to the clinical team including psychiatrist as neededIdentify, establish, and continually collaborate with community agencies, providers, and natural supports and advise team to make referrals as neededListen/respond to; interact with clients, co-workers, management, and other professionals in a non-judgmental manner.Communicate objectively, thoroughly, accurately, concisely in a manner consistent with the cognitive level of the persons involved.Demonstrate an understanding and acceptance of the organizational mission and shared purpose to assist individuals by providing quality services. Participate in efforts to enhance service provision to clients.Act and interact to build cohesiveness. Assist with building a positive image for the agency by demonstrating support and cooperation with other units and community agencies.Demonstrate professional work ethics as evidenced by overall dependability, conscientiousness, and timeliness.Ensure all necessary licenses and/or certifications remain currentOther duties as assigned
