Director of Nursing - Home HealthLocation: Bloomingdale, ILSchedule: Full-TimeSalary: Range starts at $115,000Our client, a growing and reputable home health organization, is seeking an experienced and driven Director of Nursing (DON) to lead clinical operations, ensure regulatory compliance, and drive high-quality patient care across the agency.This is a key leadership role responsible for overseeing all clinical staff, maintaining survey readiness, and ensuring the delivery of safe, compliant, and patient-centered care. The ideal candidate is both a strong clinical leader and an operational thinker who can support growth while maintaining excellence.What Makes This Role UniqueClinical Leadership with Real Impact: Lead and shape the clinical team while directly influencing patient outcomes and agency success.Survey & Compliance Ownership: Take full ownership of regulatory readiness and quality standards in a home health setting.Growth Opportunity: Join an organization focused on expansion, with the ability to build and strengthen clinical operations.Key ResponsibilitiesClinical Leadership & OversightProvide leadership and supervision to all clinical staff, including nurses and clinical managersEnsure high standards of patient care and adherence to care plansSupport and mentor clinicians to promote strong performance and professional developmentOversee clinical scheduling, caseload management, and patient assignmentsRegulatory Compliance & Survey ReadinessEnsure compliance with all state, federal, and accrediting body regulations (CMS, DOH, Joint Commission as applicable)Maintain continuous survey readiness across all clinical operationsOversee clinical documentation to ensure accuracy, completeness, and complianceLead internal audits and prepare for surveys and inspectionsQuality Assurance & Performance ImprovementDevelop and implement quality assurance and performance improvement (QAPI) initiativesMonitor patient outcomes, clinical metrics, and satisfaction scoresIdentify areas for improvement and implement corrective action plansEnsure adherence to infection control and patient safety standardsOperational & Clinical CoordinationCollaborate with operations, intake, and scheduling teams to ensure continuity of careSupport census growth while maintaining appropriate staffing and clinical qualityParticipate in care planning and case conferencesEnsure timely admissions, recertifications, and dischargesStaff Development & RetentionRecruit, hire, train, and retain clinical staffConduct performance evaluations and provide coaching as neededFoster a culture of accountability, teamwork, and professionalismAddress performance issues and implement corrective actions when necessaryPhysician & Community CollaborationMaintain strong relationships with physicians and referral sourcesEnsure effective communication between clinical team and external partnersSupport the organization's reputation for quality and responsivenessQualificationsRequired:Active Registered Nurse (RN) license in good standingMinimum 2-5+ years of leadership experience in home healthStrong knowledge of home health regulations, compliance, and survey processesExperience managing clinical staff and overseeing patient care deliveryStrong understanding of OASIS, documentation standards, and Medicare guidelinesPreferred:Prior experience as a Director of Nursing or Clinical DirectorExperience with accreditation surveys (Joint Commission, CHAP, ACHC, etc.)Strong background in quality improvement and clinical operationsSkills & AbilitiesStrong leadership and team management skillsExcellent communication and interpersonal abilitiesHighly organized with strong attention to detailAbility to manage multiple priorities in a fast-paced environmentClinical decision-making and problem-solving skillsAbility to balance compliance, quality, and growthWhy Join This Organization?Leadership role with significant impact on clinical operationsSupportive and collaborative executive teamOpportunity to grow and strengthen a clinical programStable and growing home health organizationMeaningful work improving patient outcomes in the home settingIf you are a strong clinical leader who thrives in a fast-paced home health environment and is passionate about delivering high-quality, compliant care, we encourage you to apply.Don't have your resume on hand?
