Summit Medical Group is recruiting for an Integrated Pharmacist. This is a full-time position. This position works within a multidisciplinary healthcare team, supporting identified populations of need. The Integrated Pharmacist will support ongoing efforts of Quality Metrics, MTM, and on-site collaboration with our providers. Pharmacists will be responsible for applicable quality metrics as directed by Payors. Examples of Duties (List does not include all duties assigned) * Comprehensive Medication Reviews: Utilizing third party resources, Assess all patient medications for effectiveness, safety, and proper usage. * Medication Action Plans: Creating tailored plans for medication adherence and optimization. * Patient Education and Counseling: Instructing patients on correct medication usage and health condition management. * Medication Reconciliation: Ensuring accuracy of medication lists during healthcare transitions. * Monitoring and Managing Therapy: Overseeing medication efficacy and safety, adjusting dosages, and interpreting lab tests. * Collaboration with Healthcare Team: Coordinating with other professionals for integrated patient care in person in clinic setting or telephonically. * Quality metric Management: Identifying and monitoring key performance indicators, analyzing data for trends and areas of improvement, setting quality goals, implementing and adjusting strategies for better outcomes, and fostering a culture of quality within the healthcare setting. * Actively participates in site-level quality improvement activities: Each employee will contribute to the continual evaluation site performance as well as the implementation and measurement of improvement activities that increase the quality of care provided to patients. * Maintains strictest confidentiality both internally and externally. * Adheres to expectations of the corporate compliance plan. * Performs related work as assigned. Education * Doctorate of Pharmacy (PharmD) or equivalent degree. * Have graduated from a college of pharmacy after May 1996 or passed an approved Accreditation Council of Pharmacy Education (ACPE) program that has both didactic and clinical elements. Experience * 3+ years of experience defined as a combination of managed care pharmacy experience and experience collaborating with physicians and other prescribers on medication management and improving outcomes OR graduated from an ambulatory care residency. Certification/License * Current unrestricted Tennessee licensure to practice pharmacy.
