The Grand Rehabilitation and Nursing at River Valley - ASSOCIATE NURSING DIRECTOR The Associate Nursing Director (AND) is a key clinical leader who supports the Director of Nursing (DON) and Assistant Director of Nursing (ADON) in ensuring the highest quality of resident care and regulatory compliance. This role consolidates the specialized expertise of an Infection Preventionist, Clinical Care Manager, and Wound Nurse to provide comprehensive oversight of high-risk clinical areas and 24-hour facility support. KEY RESPONSIBILITIES Clinical Care Management & Leadership• Regulatory Readiness: Lead the facility in survey readiness and participate in Quality Assurance Performance Improvement (QAPI) committees.• Serve as liaison to the unit teams, ensuring prompt escalation and resolution of clinical issues while promoting consistency in clinical practice across all units through reinforcement of standards of care, policies, and evidence-based practices.• Staff Mentorship: Supervise and evaluate nursing staff (RNs, LPNs, CNAs), providing bedside education and clinical coaching as needed. Infection Prevention & Surveillance• Program Management: Develop and maintain the facility-wide Infection Prevention and Control Program (IPCP) per CDC, CMS, and state standards.• Surveillance & Data: Conduct daily surveillance of laboratory results (e.g., MDROs) and maintain line listings to identify and investigate healthcare-associated infections (HAIs). Report findings in QAPI every month as per DOH regulations.• Antibiotic Stewardship: Lead the Antibiotic Stewardship Program to monitor antibiotic use protocols and resistance patterns.• Vaccination & Health: Organize and track immunization clinics for residents and staff (Influenza, COVID-19, TB).• Environmental Audits: Perform regular rounds to audit PPE compliance, hand hygiene, and sanitation in dietary, laundry, and housekeeping areas. Wound & Skin Care Management• Program Management- Develop and maintain the facility Wound and Skin care Management Program (WSCMP) per CDC, CMS and state standards.• Weekly Rounds: Complete comprehensive weekly wound rounds for all residents with pressure injuries, surgical sites, or ulcers.• Treatment Planning: Work alongside the Wound Care Physician and Formulate evidence-based wound treatment plans, including staging and monitoring progress via EMR documentation. Update the care plans and orders accordingly.• Reporting- report to QAPI monthly with updates regarding wounds.ON-CALL ROTATION & EMERGENCY AVAILABILITY• Rotation Schedule: Participates in a rotating on-call schedule with the Assistant Director of Nursing and other nurse managers (typically one week every 4–6 weeks or every other weekend).• Emergency Resource: Serves as the primary clinical resource person for off-hour nursing emergencies, including triaging acute resident changes and staffing crises, along with the DON and ADON• On-Site Response: Must be available to return to the facility for critical incidents such as outbreaks, unexpected deaths, or significant regulatory visits during on-call hours.QUALIFICATIONS• Licensure: Active and unrestricted Registered Nurse (RN) license in New York State.• Experience: 3–5 years in clinical nursing within a SNF, with 2+ years in management.• Certifications:o Infection Preventionist Certification (CIC, IP-BC, or LTC-CIP) required.o Wound Care Certification (WCC, CWCN) highly preferred.What We OfferWeekly PayCompetitive Compensation401(k), Health Insurance & PTOComprehensive Benefits PackageProfessional Growth & Career StabilityInnovative Training ProgramsSupportive team environmentHow to Apply:Our simple and smooth online application process makes it easy for you to apply!
