The final salary and offer components are subject to additional approvals based on Cook County Health (CCH) policy. Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at CCH. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement. This position is exempt from Career Service under the CCH Personnel Rules. LOCATION: CERMAK HEALTH SERVICES (DEPT. OF CORRECTIONS), 2800 S. CALIFORNIA AVE. DEPARTMENT: CERMAK HEALTH SERVICES SHIFT: 3PM-11PM SALARY: $60,520 - $65,217 JOB SUMMARY Under supervision, the Milieu and Group Treatment Facilitator assists with planning, developing, and implementing therapeutic services in the assigned mental health program. Orients patients and tracks patient behavior, provides and monitors patient care activities in the milieu, and provides group counseling, and care to individuals with a mentally illness or substance use disorder. Typical Duties * Conducts group and specialty counseling sessions as assigned * Leads and/or co-leads community and milieu meetings as assigned * Establishes and maintains a positive environment for learning how to be accountable for one's choices in a caring community where members can help themselves and each other make positive changes (a.k.a., therapeutic community program) * Accompanies and observes patients during recreational activities, both on and off the unit * Completes clinical documentation, including progress note charting, and staffing notes in compliance with Cook County Health (CCH), work site, regulatory and, where indicated, billing requirements. * Assists with Crisis Intervention under the direction of an independently licensed clinician * Orients new patients and tracks their adjustment to the therapeutic environment * Communicates and collaborates with other CCH staff and community providers regarding patient care issues * Maintains assigned caseload and program statistics * Maintains the clinical integrity of patient records * Attends and participates in team, administrative, and clinical staffing meetings * Attends staff development and departmental meetings as scheduled * Works with a patient to identify treatment goals and treatment progress and shares this information with the independently licensed clinician creating or updating a treatment plan. Reports physical plant and environmental problems to administration to ensure staff and patient safety * Develops and maintains with medical, nursing, administrative, and correctional staff to ensure quality patient care * Adheres to all CCH policies and procedures of the assigned work site and of Cook County Government. * Performs other duties as assigned Minimum Qualifications * Bachelor's degree in psychology or related field from an accredited college or university (Must provide official transcripts at time of interview) * Two (2) years of experience in a custodial setting providing group, mental health, and/or substance use disorder services Knowledge, Skills, Abilities and Other Characteristics * Working knowledge of psychiatric diagnoses and treatment modalities * Demonstrates excellent communication skills with patients and motivates them to cooperate * Ability to function in all clinical areas of the department such as acute care units, sub-acute care units, residential unit, intake, etc. VETERAN PREFERENCE PLEASE READ When applying for employment with the Cook County Health & Hospitals System, preference is given to honorably discharged Veterans who have served in the Armed Forces of the United States for not less than 6 months of continuous service.
