Medical Social Worker II

Shift: Working Hours: Summary:Demonstrates proficiency, autonomy and accountability in assuring patient care management needs are met from a well coordinated interdisciplinary and collaborative perspective with case manager and nursing supervisor. Maintains understanding of performance measurements. Takes leadership in Performance Improvement and Customer Service initiatives. Serves as a preceptor. Must be able to demonstrate the skills and knowledge necessary to provide intervention appropriate to the age, education level, and capabilities of the person served.Other information:Must have a Master's degree in Social Work. Must be a Licensed Master Social Worker.This job will be authorized 80.00 hours bi-weekly.
